ACAT Referral Form (aged care assessment team)

ACAT provides a service to the entire Mackay Health Service District including Bowen, Collinsville, Moranbah, Clermont and St Lawrence. The area is large and the team is only small so it is hoped that by revising the ACAT referral form, the number of unnecessary or inappropriate referrals can be reduced. The ACAT 2012 Referral form has been put together to assist with educating referrers about who is eligible for assessment and the aim is that it will be friendly and easy to use.

A non-profit organisation, Diabetes Australia works in partnership with diabetes consumer organisations, health professionals, educators and researchers to minimise the impact of diabetes in the Australian community. 

Heart Health Resources

The Heart Foundation provides up-to-date, high quality information regarding cardiovascular health to the professional community. PDFs available cover reducing risk, absolute cardiovascular disease risk assessment, absolute cardiovascular disease risk, and improving cardiovascular health outcomes. On their site you will also find clinical information, Aboriginal health, data and statistics, Australian Physical Activity Network, publications and resources and a portal to help clinicians and policy makers to access Australian guidelines.
